HOW TO ELIMINATE RECURRING RECERTIFICATIONS (UPOC) — TIPS & TRICKS
At times, a therapist may complete a recertification (UPOC) document only to find that in the track, there remains a recertification due for the same date range. This can happen to all three disciplines, but most cases occur with speech-language pathology.
Using Speech Therapy as an example scenario:
An evaluation is completed for dysphagia and speech/language/cognitive therapy. Both types of therapy are selected, which opens specific pathways within the evaluation document to be completed.
During the certification time, all dysphagia goals are met, although the SLP plans to recertify this patient for speech/language/cognitive therapy and continue that part of the plan of care. The SLP track shows that a recertification is due.
As the SLP opens the recertification document, they DE-select dysphagia from the list of Types of Therapy because these goals have been met. This should NOT be de-selected.
The recertification document is completed, and when returning to the SLP track in case manager, a recertification document still appears in red as due, even though the completed document is directly above it.
This is due to the dysphagia library pathways not having their required fields completed. Even though the goals may have been met for one, all Types of Therapy that have been checked during the POC must be checked on each document afterward. This is to satisfy the library pathway requirements and has no impact on the POC written by the SLP.
How to Correct the Repeating Recertification from the Example Scenario:
To correct this, the SLP must re-open and unlock the completed recertification document and select all the Types of Therapy referenced earlier in the POC.
They will then re-sign the document, and the library pathway will have been satisfied and the document that was due will disappear.
Remember, this is a certifying document and is signed by the physician. If the error is found AFTER the physician has signed it, it must be sent back to the physician for an updated signature. If the physician is using Clinisign, this will happen automatically.
To avoid this from occurring, the SLP should have allowed the automatically checked Types of Therapy to remain when opening the recertification document.
